Gritty and realistic, remaining true to the spirit of the universe it is set in - this has the potential to be one of the greats on this site if it can maintain that realistic feel coupled with the connection to the MC - so long as the author can avoid the pitfalls of turning Lilith into an OP character or making her path too easy - this is Warhammer after all, easy would not be believable :)

performing skin graft surgery at 8 years old! - storybookknightRoyal Road★★★★ 3.5While the core idea of a wunderkind healer in the 41st Millennium is just fine, the story's pacing and characterization is all over the place.
Lilith is in desperate need of peers - fellow soldiers, fellow medicae, people she can be friends, enemies, and acquaintances with.
As it stands, the action and drama feels like it's unconnected to the narrative - there's no stakes when soldiers are at risk, because Lilith hasn't spoken to a single grunt. There's no sense of victory when she succeeds at a complicated surgery, because there's no rival medics looking on in jealousy or friends who were on the verge of losing hope.
Also, the story is missing a sense of verisimilitude when it comes to actual army operations - the "95% boredom, 5% sheer terror" of a soldier's life could be a bit more on display; Lilith should be going on patrols, standing watch, and all the other nonsense of a soldier's life, experiencing character interactions during downtime, then with things getting slowly worse as the Orc threat (or whatever threat is next) nears.
If/once Lilith survives the current arc, that's more of what I'd like to see. Assign her to a squad, give her squadmates, give the reader a taste of "a day in the life of an Imperial Guardsman" before things get hairy again. - KittyJinglesRoyal Road★★★★ 3.5A drama about identity and faith as we follow a guy adjust to being a WH40k orphan girl.
Story is a lot of dialogue, there is some action. A lot of the tension is about her doubt and questioning, will they discover she's an isekai and say she's a heretic. What should she do. Etc. The writing isn't the tightest (tense changes, repetition/echoes) and it's structure is kind of a meander through one identity crisis after another, but it's not bad for fanfiction
There's heartfelt prayer and day to day life in 40k setting. If you're into WH40k and acceptance and hugs and found family you'll be into it - SalemWTRoyal Road★★★ 3.0Read until chapter 20? Or so I could tell the main character was already developing into a Mary sue, and I didn't like how much the other characters were marveling at the character with every opportunity. Very contrived, no hate just honest opinion. Need more words to finish writing this review.

The gender bent MC quickly becomes subsumed into the setting and we get to have a relatively good time seeing one of the rarely explored parts of the setting. The basis of the Imperial Faith and its impact on that setting is usually not explored in any nuance and that interesting in its own way.
The downfall is the Main Character, as 'Lilith', rapidly goes from orphan to the local Saint without any real explanation. She's 'Gifted', she's so clever questioning the Imperial Truth is ok, she's the one orphan every member of the orphanage love unconditionally, when she leaves the room everyone talks about her because shes the center of the world. The last event that made me drop the story is the unveiling that apparently the MC has 'super medic powers' that cause a battlefield medical ward to sit with bated breath as the 13 year old does surgery during an Orc Waaagh.
I know the story is a story about trying to make a Medic an important part of the setting, but it just reads like a Mary Sue found Faith. - ShoultzRoyal Road★★★ 2.5Good start but the description of this fanfic isn't as described. Main character turns into an extremely powerful psyker and also a blank. So basically a mary sue, so if you're looking for a story about a regular guardsmen patching up soldiers like the description, this isn't it. I'm really disappointed too because there was no built up really and this is just revealed in a single chapter.
