Through the Cracks

Isaak was not exactly the happiest guy.  Stage-4 melanoma will do that to you.  Not content with ravaging his body it had begun infiltrating his brain, ruining his concentration and slipping him into a bad trip.  And on top of that, among the hallucinations, he accidentally stumbled and fell.
