This isn't just a retelling of Arcane with a character insert, this is a brand new story that follows its own sequence of events of what could've been, chasing down a spiralling path of butterfly effects, some leading to good outcomes, some to bad, but every action and it's consequence feels like a natural progression of events, both believable and plausible.
The character insert, Garou, has been written consistently and in-line with his narrative purpose, that is, an anti-hero that you wouldn't be remiss if you found yourself rooting for him, though you'd still find him willing to commit villainy here as he did in OPM. Regardless, his inclusion is written strong enough that you wouldn't need to know anything about OPM or Garou to read this and still fully enjoy it.
Overall, it's a great read if you've watched Arcane, and if you haven't watched it, go watch it and then come back and read this to heal the emotional baggage that the show left on all of us.
